공기나 복벽의 방해 없이 췌장을 보다 자세히 관찰하고자 하는 목적으로 처음 개발된 내시경초음파(endoscopic ultrasonography, EUS)는 현재 췌장뿐 아니라 다양한 소화기 질환에서 유용한 진단 및 치료 도구로 사용되고 있다. 초음파가 내시경 종축의 수직방향으로 발사되는 방사형 내시경초음파(radial echoendoscope, radial EUS)를 이제까지 주로 사용하여 왔으나 내시경의 종축과 같은 방향으로 초음파가 주사되는 선형배열 내시경초음파(linear-array echoencoscope, linear EUS)가 개발된 이후에는 실시간 조직검사를 포함한 여러 가지 중재적 시술이 가능하게 되면서 EUS의 활용분야는 더욱 다양해 졌다. 본 종설에서는 현재까지 알려진 linear EUS의 활용과 새로운 시도들에 관하여 전반적으로 기술하고자 한다.
