Bisphosphonate-related osteonecrosis of the jaw (BRONJ) has been increasing every year, as the number of elders receiving bisphosphonate (BPs) therapy is increases. It is believed that BPs enhance the ORN by inhibition of the osteoclastic activity, but the exact mechanism is poorly understood. Patients who had received long-term oral BP therapy had a high recurrence rate of BRONJ. They needed to undergo segmental mandibulectomy and reconstruction, beyond getting just the conservative treatment, such as sequestrectomy. We present 2 such cases in this report. The patients had 7 years and 13 years of oral BP administration, respectively. These cases are being reported as we had good results after reconstruction, using reconstruction plate subsequent to segmental mandibulectomy.
