Posterior interosseous nerve (PIN) syndrome is a rare compressive neuropathy of the forearm involving the deep radial nerve. Etiological origins of spontaneous PIN palsy are diverse and include entrapment neuropathy, space-occupying lesions, inflammatory change, neuralgic amyotrophy, mononeuritis and fascicular constriction. We present a case of non-traumatic spontaneous PIN syndrome with sonographic imaging, magnetic resonance (MR) imaging findings and electrodiagnostic correlations. This is the first to comprehensively report on the sonographic and MR imaging findings of a spontaneous case of PIN syndrome.
