Phrenic nerve palsy (PNP) has previously been associated with brachial plexus injury (BPI) with root avulsion. The incidence of phrenic nerve injuries associated with traumatic BPI ranges from 10 to 20 %, but it is overlooked because unilateral injuries are frequently asymptomatic. Rarely, unilateral PNP have been reported as one of unrecognized causes of dyspnea, exercise intolerance and even respiratory failure, especially in patients with underlying pulmonary diseases. We hereby report a case of bilateral BPIs with unilateral PNP, diagnosed by proper evaluation tools, who suffered from early respiratory complications such as prolonged ventilator weaning and recurrent pneumonia with pleural effusion, that was eventually managed by comprehensive pulmonary rehabilitation. To our knowledge, this is the first report in Asia where clinical significance of unilateral PNP is described in a patient with traumatic BPI.
