배경 : 급성관상동맥증후군(acute coronary syndrome, ACS)이 의심되는 환자에서 기존에 사용 중인 검사방법들로는 심근허혈의 진단이 종종 어려운데, 최근 심근허혈 초기를 예민하게 진단할수 있는 표지자로 ischemia modified albumin (IMA)가 개발되었기에 ACS 진단에 있어서 기존의 심장표지자인 CK-MB 및 cTnI와 함께 새로운 허혈 표지자인 IMA의 진단적 유용성을 평가하고자 하였다.방법 : ACS가 의심되는 87명의 환자를 대상으로 하였고, 심장전문의에 의해 내려진 최종 진단에 따라 ACS군(n=27), 뇌혈관질환군(n=11) 및 non-ACS군으로 분류하였다. 이들 환자들에서측정된 IMA, CK-MB 및 cTnI 결과들을 이용하여 receiver oper-ating characteristics (ROC) curve 분석 및 ANOVA 및 다중비교 등을 시행하였다. 결과 : ROC curve 분석상 IMA는 CK-MB나 cTnI보다 예민하나 특이도가 떨어지는 것으로 나타났으며, 특히 음성예측도가가장 우수한 것으로 나타났다. 또한 ACS 진단을 위한 최적의cutoff치는 110 U/mL로 나타났다. 특히, 증상 발현 후 4시간 이내에 측정된 IMA는 심전도나 기존의 심장표지자로는 진단이 불가능했던 ACS 환자까지 포함해 100% 발견율을 보였다. 그러나IMA 양성 소견을 보이는 환자의 반수는 ACS가 아닌 뇌혈관질환(주로 허혈성 뇌졸중), 암, 외상, 지방색전증 등의 다른 질환으로 밝혀졌으며, IMA 음성인 환자들의 대부분은 ACS 환자가 아급성관상동맥증후군에서 Ischemia Modified Albumin 의 임상적 유용성 311닌 것으로 나타났다.
결론 : IMA는 ACS의 조기 진단에 유용한 초기 표지자로서, 특히 심근경색으로 진전되기 전이나 심근경색을 동반하지 않은 심근허혈상태로 심전도나 기존의 심장표지자 결과가 정상인 ACS환자를 찾아내는데 유용한 것으로 나타났다. 그러나 IMA 양성결과가 허혈이 발생한 조직을 특이적으로 나타내는 것은 아니므로, 증상 발현 초기에 IMA 결과가 음성인 경우 ACS 진단을 배제하기 위한 용도로 유용하게 사용될 수 있다.
