내시경적 역행성 담췌관조영술(ERCP)은 췌장과 담도 질환의 진단뿐만 아니라 치료에도 유용한 검사이지만, 다른 상부위장관 내시경 검사에 비하여 상대적으로 합병증의 발생위험이 높아 시술에 주의를 요한다. 내시경적 역행성 담췌관조영술을 시행한 후 췌장염, 담관염 등이 비교적 흔하게 발생하며, 시술에 관련된 출혈이나 천공도 보고되고 있다. 내시경적 역행성 담췌관조영술과 관련한 출혈은 십이지장 유두괄약근 절개술 후 발생한 담관내 출혈이 대부분으로 담관외 출혈은 드물지만 치명적일 수 있으므로 적절한 진단과 신속한 치료가 필요하다. 저자들은 담관 결석을 동반한 급성화농성 담관염 환자에서 내시경적 역행성 담췌관조영술을시행한 후 발생한 피막하 간혈종을 경험하여 이를 보고하는바이다. 간내 피막하 혈종은 매우 드문 합병증이나 내시경적역행성 담췌관조영술 시행 후 점진적인 복통을 호소하는 환자에서 감별해야 할 질환이다.
