Introduction : Ticlopidine hydrochloride is an antiplatelet agent frequently used in the patients with cerebrovascular disease. Its uncommon but serous hematological complications include thrombotic thrombocytopenic purpura (TTP), pancytopenia, and aplastic anemia. We describe 2 patients with serious hematologic complications due to ticlopidine use. Case report : A 78-year-old woman presented with drowsiness, nausea and scattered purpura on both thighs. Six weeks before, she had suffered from cerebellar infarction and had been given ticlopidine therapy. She was diagnosed as having TTP. She was managed with steroid therapy but died in 4 days. After receiving ticlopidine, a 76-year-old woman developed dizziness, light headache and ileus. She showed pancytopenia, which improved discontinuing ticlopidine medication. Besides these cases, we reviewed 35 cases from published articles (15 cases of TTP, 20 cases of pancytopenia or aplastic anemia). Conclusion : We should be aware of this potentially fatal but treatable complication of ticlopidine therapy. Korean Journal of Stroke 2000;2(1): 112~116
