Background : Impairment of upper extremity function contributes greatly to functional disability after stroke. The remission of upper extremity function takes place mainly about the time in the first 3 months, but previous study did not make clear detailed knowledge of the time course of recovery. The purpose of this study was to analyze recovery pattern of upper extremity motor function in stroke survivors. Methods : We weekly evaluated total 51 stroke patients for deep tendon reflex(DTR), spasticity, and voluntary motor function. DTR was rated on a four-point scale (0: not evoked, 1: hypoactive, 2: ormoactive, 3: hyperactive). Spasticity was assessed with modified Ashworth scale. Voluntary motor function was graded on three-point scale (0: no observable motion, 1: active motion with synergy, 2: active motion without synergy). Results : DTR was evoked in the biceps and triceps and developed in the finger flexors after appearance of brachioradialis jerk. Spasticity was firstly noted in the shoulder adductors and elbow flexors, and then also demonstrated in the wrist and finger flexors. However, voluntary motor recovery was detected in the shoulder adductors and elbow flexors similarly, and was followed by that of shoulder abductors and elbow extensors. Recovery time of wrist and finger voluntary motor function significantly correlated with development of spasticity of shoulder adductors. Conclusions : Evolution of deep tendon reflexes and spasticity followed proximal-distal principle. However, recovery of voluntary motor function was more apparent in the muscles contributing to flexor synergy and showed a trend of plateau confined to proximal muscles.
