As stroke composes of various distinctive mechanisms, it is imperative to clarify different stroke mechanisms and etiologies in each stroke cases. In year 2009, new insights into etiology and stroke mechanisms were provided in the field of lacunar infarction and risk factors of stroke. However, anticipated genome-wide association studies were resulted to have subsequent limitations in validity and reproducibility. More sophisticated equipments and methodology are warranted to investigate stroke etiology and mechanisms in the future. (Korean J Stroke 2010;12:43-47)
