Cerebral fat embolism is a potentially lethal complication of trauma or surgery on the lower extremity.
Neurological manifestations are variable and clinical diagnosis is often challenging. A 58-year-old man developed acute confusion 40 hours after an uneventful unilateral total knee replacement arthroplasty. Diffusion-weighted MRI of the brain revealed multiple small high signal intensity lesions on both hemispheres which were not apparent on T2-weighted images. After a sudden respiratory failure that occurred during intensive management, the patient remained stuporous. In this patient, diffusion-weighted images were important in diagnosing cerebral fat embolism in acute stage. Among the patients who show altered mental status after total joint replacement arthroplasty,
a cerebral fat embolism syndrome must be considered as one of the differential diagnosis.
