Infrequent infarction in the territory of the medial and lateral branches of the posterior choroidal artery (PChA) involving the choroid plexus may be associated with multiple branches and rich anastomosis. A fifty-four year old man presented with acute onset of non-vertiginous dizziness and left visual field defect. Diffusion weighted MRI of the brain showed hyperattenuated lesions in the hippocampus, the parahippocampal gyrus, the tail of the caudate nucleus, the lateral geniculate body, the medial side of the thalamus and the choroid plexus that are supplied by the right medial and lateral posterior choroidal arteries. We report a case of infarction in the territory of the medial and lateral branches of the posterior choroidal artery (PChA) involving the choroid plexus which is demonstrated by multimodal brain MRI.
