Cerebral vasculitis may occur in patients with systemic lupus erythematosus (SLE), but it usually occurs several years after the diagnosis of SLE. A 27-year-old woman was presented with dysphagia and left Horner syndrome. Diffusion weighted MRI showed subtle hight signal intensity in the posterolateral aspect of the left medulla oblongata. Four-vessel cerebral angiography showed no specific abnormal finding. We prescribed an antiplatelet agent under the impression of cerebral infarction presented with lateral medullary infarction. On the third day of admission she had a fever of 38 ℃ and complete blood counts showed leukopenia and anemia. Later, she was diagnosed as having SLE supported by laboratory results. We report a case of SLE presenting as lateral medullary infarction, which might have been induced by vasculitis.
