Infarctions of cervical or thoracic spinal cord have been reported, but infarction of the conus medullaris is rare. We report a patient diagnosed as an infarction of the conus medullaris. A 72-year-old man presented a sudden onset of mild paraparesis with acute urinary retention. Follow-up magnetic resonance imaging (MRI) of the spinal cord showed abnormal signal intensity in the conus medullaris. This case demonstrates the clinical importance of infarction of the conus medullaris in differential diagnosis of a patient who suddenly had paraparesis and urinary retention as well as diagnostic usefulness of follow-up MRI when the infarcted lesion of the conus medullaris did not shown on initial MRI of the spinal cord.
