Background: The purpose of this study is to investigate the usefulness of flow volume (FV) measurement by Duplex ultrasono-graphy (DUS) parameters in patients with acute stroke. Methods: The FV of extracranial cerebral arteries, calculated as as the pro-duct of flow velocity and the cross-sectional area of each vessel by using DUS in patients with acute stroke. The anterior circulat-ing flow volume (ACFV) was determined as the sum of the FV of the internal carotid artery (ICA), total cerebral flow volume (TCBFV) as the sum of the ACFV and the vertebral artery of both sides. The FV of each artery and TCBFV were compared accord-ing to severity of stenosis (≥70% versus <70%) and between preserved and impaired cerebrovascular reserve (CVR) measured by Single Photon Emission Computed Tomograpy (SPECT). Results: Although the FV of the ipsilateral ICA FV was decreased in more than 70% of stenosis, there was no significant differences of TCBFV. In the impaired CVR group, however, the FV of the ipsilateral ICA, ACFV, and TCBFV were decreased than in the preserved CVR group. A TCBFV of more than 546 mL/min predicated preserved CVR with 69.5% sensitivity and 71.0% specificity. Conclusion: In this study, we demonstarated FV by using DUS, correlated with CVR in patients with ischemic stroke, which is helpful to elucidate the hypoperfusional con-tribution to the stroke occurrence and to understand the hemodynamic status.
