Stroke is a common cause of death in Korea. The concept of acute intervention has been emphasized in ischemic stroke. Partic-ularly, technical advances and the enhanced availability of imaging technic have permitted new challenges of thrombolysis based on the selection of ‘tissue at risk’ of brain after acute ischemic stroke. This article present a review of clinical trials for acute treatment of stroke introduced in 2008. (Korean J Stroke 2009;11:56-61)
