A 41-year-old man was presented with dysarthria, left side hypethesia, and right side ataxia. He has suffered from recurrent oral and genital ulcer since 10 years ago. T2-weighted MRI showed a high signal intensity lesion involving the dorsolateral medulla and extending to the upper pons. We prescribed a platelet antiaggregating drug under the impression of cerebral infarction presented with Wa l l e n b e rg syndrome. Six months later, horizontal diplopia developed suddenly. On follow-up MRI, a newly developed high signal intensity lesion was found in the paramedian midbrain area, but the previous lesion in the medulla disappeared. Cerebrospinal fluid examination tapping showed pleocytosis and elevated protein level. We treated him with corticosteroid and his symptoms improved 3 months later. Neurobehcet’s disease was suggested as a diagnosis of this patient and this is the first case report of neurobehcet disease presented with Wallenberg syndrome.
