Background: The aim of this study was to compare the effects of EMG-triggered functional electrical stimulation (EMG-FES) with FES on cortical excitability and functional improvement with hemiplegic patients. Methods: Eight subjects who had stroke at least 6 months prior to this study and were able of extension of their hemiplegic wrist were enrolled in this study. They received rehabilitation programs using the EMG-FES and FES at the interval of 2 weeks. The effects were assessed by Box and Block Test and transcranial magnetic stimulation (TMS) studies. The efficacy of EMG-FES and FES on cortical excitability and clinical improvement was compared in an analysis of Wilcoxon signed ranks test, respectively. Results: Intracortical inhibition(ICI) was significantly reduced after the EMG-FES (p=0.049). And the score of Box and Block Test was significantly increased after the EMG-FES (p=0.042). However, there was no significant difference in the other TMS studies and Box and Block test after the FES. Conclusions: The rehabilitation programs using EMG-FES may be more effective than those using FES in subjects with hemiplegic hand. The clinical improvement might be developed by disinhibition of cortical excitability following rehabilitation programs using EMG-FES. (Korean Journal of Stroke 2005;7(2): 159~164)
