The term “accessory middle cerebral artery (MCA)” is used when the anomalous artery arises from the anterior cerebral artery. We report two cases of the MCA territorial infarction with an accessory MCA. The presence of an accessory MCA may contribute to the topographic diversity of the MCA territorial infarction. (Korean J Stroke 2008;10:51-54)
