목적: 경피적신절석술(Percutaneous Nephrolithotomy) 예정 환자 중 경피적신루설치술(Percutaneous Nephrostomy) 의 Pig tail catheter에서 Straight catheter로 변경 후 경피적신절석술(Percutaneous Nephrolithotomy)의 시술관계자에게 적합성을 알아보고자 하였다.
대상 및 방법: 2005년 1월부터 2015년 12월까지 경피적신절석술(PercutaneousNephrolithotomy)을 184건을 시행하였다. 2011년 10월 기준으로 하여 이전에는 경피적신루설치술(Percutaneous Nephrostomy)시 Drainage catheter를 Pig tail catheter로 시술하였고, 2011년 10월 이후에는 Pig tail catheter에서 Straight catheter로 변경하여 시술하였다. 그에 따라 경피적신절석술(Percutaneous Nephrolithotomy)의 시술관계자에게 설문조사를 실시하였다. 설문대상자는 비뇨기과 시술관계자의 10명으로 대상하였다.
결과: 2011년 10월 이후 경피적신절석술(Percutaneous Nephrolithotomy)을 받은 환자가 145건으로 급증하게 되었고 Pig tail catheter는 43건, Straight catheter는 102건으로 시술하였다. 설문지 결과로써 Straight catheter로 변경 후 만족도 조사의 평가로는 “매우 만족”은 5명, “만족”은 5명으로 “불만족”은 없는 것으로 조사되었다. 만족도 세부사항으로 Wire pass 용이 부분이 5점 만점에 4점으로 높게 나왔고, 그 다음으로 Tract dilatation 용이와 수술시간단축은 중간점수인 3점보다 조금 높게 나왔다. Straight catheter로 변경후에는 수술 중 Wire가 빠지는 경우가 없어졌다는 전문의의 기타 의견도 있었다. 불만족부분의 기타의견은 없었다.
결론: 경피적신루설치술(Percutaneous Nephrostomy)에서 Straight catheter로 위치 할 경우경피적신절석술(PercutaneousNephrolithotomy)의 시술자는 유도철사가 요관 내로 진입하기 용이하게 되었고, 통로 확장도 편리하게 되어서 수술의 성공률을 최대로 높이고 합병증을 최소화시킬 수 있을 것으로 생각된다.
