Asymmetric loading between the medial and lateral compartments of the knee was associated with the development of knee osteoarthritis. Knee internal contact force could be accurately measured using instrumented total knee replacement, which is the primary standard for the validation of knee contact force estimation. In our previous study, a contact force estimation methods using reaction element was developed and validated. However, the previous estimation model has geometrical limitation such as muscle attachment location because a template human model was used. In this study the medial and lateral knee contact forces were estimated using a patient specific human full body musculoskeletal model with two-step methods by using model-A and model-B. Firstly, thigh and shank of model-A were developed according to patients anatomy. With motion capture data, the model-A estimated medial and lateral contact forces. Secondly, errors of model-A were investigated and model-B was developed to revise model-A. For the model-B, knee joint assumption were set to 3-degrees of freedom and marker positions were corrected to match the patients frontal x-ray image. Mean values of root mean square error of medial and lateral contact forces for model-A were 176N, 232N and for model-B were 194N, 151N, respectively. Mean values of coefficient of determination of medial and lateral contact forces for model-A were 0.42, 0.23 and for model-B were 0.42, 0.65, respectively.
