In recent years, modern people with aging and wrong dietary habits, physical activity due to decreased prevalence of chronic disease called lifestyle diseases have increased. Despite the development of medical technology, which of these chronic diseases cure rate is significantly low. In this paper, using smart phones and the web, the development of chronic diseases, healthcare systems, and through it the patient"s BMI (Body Mass Index), blood glucose, blood pressure readings to the server via the server by typing the information stored in the database and the patient numbers that correspond to the information entered by the prevention and related news, health information, and connect to the database. In addition, numerical input via mobile devices, health information can be exchanged. Through this rapidly growing chronic health care for patients who can be tailored for each individual. Furthermore, Ubiquitous healthcare environments are reviewed to achieve.
