광범위 회전근 개 파열 환자에서 적절한 보존적 치료에도 호전되지 않는 경우에는 수술적 치료를 고려하게 되며 최근 역형 견관절 치환술이 그 하나의 대안으로 사용되고 있다. 하지만, 소원근까지 파열되어 능동적 거상 및 외회전이 불가능한 광범위 회전근 개 파열에서 역형 견관절 치환술을 시행할 경우 능동적 외회전은 향상이 되지 않아 수술 이후에도 일상생활에 불편함이 남게 된다. 이런 경우 역형 견관절 치환술과 광배근 및 대원근 이전술을 동시에 시행하면 능동적 거상과 외회전력을 복원할 수 있다. 저자들은 이런 술식으로 관절운동 범위 및 기능면에서 양호한 결과를 보인 증례를 경험하였기에 보고하고자 한다.
