Physical therapies have a important role of several therapeutic methods for muscle disorders, which are used to decrease pain. inflammation, nociception and spasm, and used for modulation of circulation. Therefore, it is important to understand and apply physical therapies for treatment of muscle disorders with apprehension about each diseases. We will propose possible mechanisms of physical therapies for muscle disorders,
There are some muscle disorders on masticatory muscle tissues such as myofascial pain, myospasm, muscle contracture, myositis, local muscle soreness. Among these disorders, myofascial pain is a regional myogenous pain condition characterized by local areas of firm, hypersensitivite bands of muscle tissue known as trigger points, which is induced by severe local hypoxia and tissue energy crisis.
Cryotherapy, the therapeutic use of cold in the management of injuries and painful conditions, can he effective on vasoconstriction, decreased tissue hypoxia, which helps to decrease secondary tissue damage, and decreased pain. Ultrasound is assumed to have thermal and non-thermal effects resulting in an increased metabolism, circulation, extensibility of collagenous tissue and tissue regeneration. Beneficial effects of ultrasound for the patient with myofascial pain may include improvement of pain and range of motion. EAST, which is performed by noxious stimulation on specific site, can reduce a pain condition by increasing endorphin in circulation. Dry needling at trigger point induces the mechanical distruption of trigger point, local hemorrhage and inflammation, which can be effective on removal of the nerve-sensitizing substances, decreased pain, relaxation of contracted muscles and interruption of the central feedback mechanism.
Most of mechanisms of physical therapies for myofascial pain disorders were generally verified from investigation of medical circles. And dentistry bas been simply applied therapies to masticatory muscles disorders of orofacial region. Therefore, we suggest that further researches are necessary to study how each therapy affects physiology and pathology of masticatory muscles.
